How to bulk erase a 16f886 PIC

I had a working program, I made some small changes to the code, not fuses, when I now true to program the chip, I get verification errors. The compiler is 4.124 and CCS load 4.023. I suspect the code protect bit somehow got set and is giving me the errors. Thank in advance.

I used ICD-64 and ICD-40 both have same problem, Also tried on 2 working boards.

If running under 5 volts and you can't erase and program, apply 5 volts and all seems to work normally.
