Trouble programming 16f1824 with ICD-S40

It doesn't even start to program, it gives Error Download failed - Chip Detect.
When i start CCSLOAD.exe and click the Test button it reads Device ID 0102 (wrong) if i click it again it reads 2743 (correct). Every time i click the Test button the Device ID jumps from right to wrong and back.
From the Diagnostics tab when I click 'read device id' it never reads the correct id.
I'm using CCS PCWHD 4.114 and CCSLOAD 4.27.16.21
Tried it also with CCSLOAD 4.28.18.11 but still the same results.
I am limiting the Vpp voltage with LM431 as in the pic16f1824 datasheet. The PIC is in a breadboard connected only to the ICD. I tried putting a 10k resistor between MCLR and VDD - same results. I tried all of the above with several PICs from the same batch but still no luck. Any suggestions?
