What are all the known gotchas with in circuit debugging?

Is there is a list of known problems or workarounds for in circuit debugging with ICD-U40?

For instance: What does it mean when I pause program execution (Halt Program) and the program counter reads: PC=FFFFFF

Also I have a working program that will stop responding until i (Halt Program) and (Run) and then it responds again?
