I want to use pin A6 for IO,
on the PIC18F258.

Inside PIC18F258.h, there is:

#define PIN_A0 31744
#define PIN_A1 31745
#define PIN_A2 31746
#define PIN_A3 31747
#define PIN_A4 31748
#define PIN_A5 31749

but no:

#define PIN_A6 31750

Is there any reason CCS did not include pin A6? Is there any reason I should not add it?

The only reason, is because it is not available in normal configurations (you have to be using ECIO). I have used it fine.

How do you setup PIN_A6 once you place your 18Fxxxx in EC_IO?

just add a define like the one mentioned above?

maybe to whatever file you're working on?

I tried that and it does compile fine this way.. but I haven't tested it yet.
